SUSAN is the City of Lubbock’s substance use disorder network for community health workers. SUSAN has two dedicated Case Managers and two dedicated Community Health workers to assist the public in Region 1. This program is designed for male and female Texas Residents with substance use disorders. The goal is to screen and address social determinants of health and provide harm-reduction methods and tools to help others live a safer, healthier lifestyle. SUSAN provides syringe cleaning kits, wound care kits, hygiene kits, and overdose reversal kits including Naloxone and pre-exposure Prophylactic (PREP) education information. Susan offers assistance with residential and outpatient treatment, which is determined by screening for substance use disorder. Assistance can also be provided for sober housing, transportation needs, and help with prescription medications, vision, hearing, clothing, employment, and educational needs. SUSAN employees understand that there are no two people alike, so each plan is individualized to address the needs of that particular person. Services Provided:

Screening and Assessments: StarCare’s dedicated, licensed staff provide outreach, screenings, assessments, and referrals to those who are actively seeking substance use services and/or support. The OSAR team conducts verbal screenings to gather information pertaining to the severity of use and how use has impacted day-to-day functioning or mental health to provide appropriate referrals to recovery resources.
-Support: OSAR counselors are available for additional support and case management until the individual successfully enters their referred treatment service. Our OSAR counselors also remain as a point of contact for future recovery support needs. 4 OSAR Region 1 -For uninsured or underinsured individuals, the OSAR Team can determine if you are approved for full or partial financial assistance toward your referred treatment services. (Federal and state admission priority levels):
• -Pregnant, injecting individuals (Federal)
• -Pregnant individuals (Federal)
• -Injecting drug users (Federal)
• -Individuals at high risk for overdose
